Homebound instruction is one of the services available to students ages 3 to 21 from their local school district, who, because of their condition, are unable to attend school safely or adequately. This includes students who require instruction: 1) as a result of hospitalization (homebound to be given after hospitalization) or 2) in the home as a result of a temporary medical condition which prevents their school attendance.
In order to receive homebound instruction for student:
- The parent must sign a Release of Information. The school district must obtain a completed homebound application form the attending physician as to the student’s diagnosis, and reason for confinement, and approximate length of time the student needs services.
- A recommendation for homebound must be for an extended absence, more than 3 weeks.
School personnel must notify the Student Services Administrative Assistant at (636) 851-4007 as soon as possible once they learn of a student who is going to meet these criteria.
- Services are provided only from the date the application is signed by a physician.
- It is the responsibility of the home school to secure a teacher to provide homebound instruction for students.
Homebound application forms will be distributed by the Student Services Administrative Assistant, Annette Bradley, in the Administration Building (636) 851-4007, Fax: (636) 851-4094.
